A chronic medical condition is a condition that has been (or is likely to be) present for six months or longer, for example asthma, diabetes, cardiovascular diseases, stroke, obesity and osteoporosis. The CDM is designed for patients who require a structured approach including those requiring on-going team based care from a GP and two other health / allied care providers.
A health assessment is conducted to learn whether there are any underlying health conditions that could be prevented or managed to improve quality of life and overall health.
Adults aged 40 – 49 years are encouraged to have a health assessment to evaluate your risk of developing a chronic illness. Speak to one of our doctors to have a health assessment booked.
Adults aged 75 years and over and encouraged to have annual health assessments.
